Arterial stiffness increase reflects early vascular againg and predicts future cardiovascular(CV) disease, independently of conventional risk factors in diverse clinical settings (Vlachopoulos C et al: J Am Coll Cardiol, 2010; 55:1318-1327). Carotid-femoral pulse wave velociy(PWV) is now considered the gold standard for its measurement. However, augmentation index and pressure was proven its independcy of CV events only in elderly females with end-stage renal disease and coronary artery disease. In this community-based study among Taiwanese (Wang K-L et al, Hypertension. 2010;55:799-805), they found that a transit time-independent measure of reflected wave magnitude predicted 15-year all-cause and CV mortality in men and women independent of arterial stiffness. This finding is highly valuable because it was proven in low event cohort with CV mortality in Asian general population. However, we still wants more data to shed light on better index (AI : reflection magnitude : PWV) for detection of early vascular aging and also reversal of intermediate phenotype (eg.
